- Photocopy of ID Proof (Driving license/Aadhar Card/Rashan Card)
- 8 Passport size Photographs
- Migration Certificate (in case education from other states)
- Character Certificate from the last institution attended
- Caste certificate (if applicable)
For B.Pharm. Courses
- Original mark sheet of 10th & 12th
- Original of CET or other qualifying examination Score Card(if any)
- Photocopy of ID Proof (Driving license/Aadhar Card/Rashan Card)
- 8 Passport size Photographs
- Gap Certificate (if there is any time gap from the last education)
- Migration Certificate (in case education from other states)
- Character Certificate from the last institution attended
- Diploma in Pharmacy Certificates (FOR LATERAL ENTRY ADMISSION ONLY)
For M.Pharm. Courses
- Original /Provisional B.Pharm. Degree with Detailed Marks Card
- Original GPAT Score Card (if any)
